This is the official nomination form for JMU's partner institutions to nominate their students to the exchange program at James Madison University. Please note that we will be asking you for the following information, should you wish to gather this information from your student(s) before you begin:
  • First (given) and Last (family) names, as listed on their passport
  • Gender (male/female/other)
  • Date of Birth
  • City and Country of Birth
  • Country of Citizenship and Country of Legal Permanent Residence
  • Home address and phone number (mobile or home)
  • Student's personal email address
  • Exchange term at JMU (e.g., Fall or Academic Year)
  • Student's area of study/major
