The Office of Equal Opportunity (OEO) administers the university’s policy 1324 on procedures for complaints of alleged incidents of discrimination, harassment and retaliation.  This complaint form is designed to gather the necessary information for the OEO to conduct an initial evaluation. This complaint form is sent directly to JMU’s Office of Equal Opportunity, unless this is a report of sexual misconduct such as sexual harassment, sexual assault, dating violence, domestic violence, stalking and non-consensual relationships.  Reports of sexual misconduct will be sent directly to the Title IX office.  For more information on Title IX, contact the university’s Title IX office at (540) 568-5219 or titleix@jmu.edu.

In filing this complaint, you will be referred to as the complainant.  The person against whom the complaint is made will be referred to as the respondent.  If you are a third party – complaints may also be submitted by third parties, but they will not have the rights of a complainant, see policy 1324 section 6.1.a for details.  Additionally, if you would like to make an anonymous report, you may use the Not @ JMU form.
