BRCC to JMU Co-Admission Program FERPA Release Form
James Madison University (JMU) is excited to partner with Blue Ridge Community College (BRCC) to allow invited students to be jointly admitted to BRCC and JMU. Students are admitted to degree-seeking status at JMU following successful completion of BRCC to JMU C0-Admission Program requirements. To facilitate the program, JMU and BRCC will need to share academic educational record information with each other.
With your approval, JMU will share the following information with BRCC:
· Basic student information (e.g., name, JMU ID number, JMU and personal email, local JMU mailing address, cell phone, etc.)
· Student application material, including: high school transcripts; all dual enrollment/college transcripts; SAT/ACT/AP/IB/CIE/CLEP scores; military transfer credit information
· Program status information for students, including: assigned JMU advisor; status regarding transition to degree-seeking status at JMU; program continuation/discontinuation information
· Student withdrawal/Program removal information
· Student conduct information as permitted by law
BRCC will share the following information with JMU:
· Basic Student Information (e.g., name, BRCC ID number, BRCC campus and personal e-mail, major, cell phone, etc.)
· Class schedule
· Grades at the midpoint and end of each semester
· Official transcripts for students
· Outstanding admissions requirements
· Student withdrawal/Program removal information
· Student conduct information as permitted by law

Consistent with the requirements of the Family Educational Rights and Privacy Act (FERPA) of 1974, I authorize James Madison University and Blue Ridge Community College to release my academic educational records (listed above) to each other to facilitate my participation in the BRCC to JMU Co-Admission Program. I understand that I have the right to rescind this release of my academic records at any time by notifying the Registrar at BRCC. In such an event, JMU and BRCC will make no further disclosure of my academic records without my consent which could delay or terminate the BRCC to JMU Co-Admission Program. I have read the FERPA statement and authorize the release of my educational records described above between James Madison University and Blue Ridge Community College.
